Golden Era Transition Support, LLC provides personalized guidance and compassionate care management services for older adults and their families. Founded with a commitment to enhancing quality of life during the golden years, I create individualized support plans that address daily living needs, care coordination, home safety, and long-term planning. My goal is to reduce caregiver stress, promote aging in place, and support each client’s independence with dignity and respect.

RACHEL HUDSON, FOUNDER & OWNER

With a Bachelor of Science degree in Psychology and a lifelong passion for geriatric care, I pursued and completed a Geriatric Care Management (GCM) program. As a proud member of the prestigious Aging Life Care Association (ALCA), I established Golden Era Transition Support, LLC in Southwest Michigan.  I believe that our golden years, when met with support, understanding, compassion and care, can be some of the most wonderful years of our lives. 


My services are available in Mattawan, Portage, and the SW suburbs of Kalamazoo. If your Senior is located outside of this service area, please feel free to reach out for consideration.
